Welcome to Astute Counseling & Wellness Services! Please complete the following form to help us understand your needs and match you with the right therapist.

Friday: 9:30 AM – 5:30 PM
Saturday 9 AM-5 PM (wellness appointments only)
Please note, we continue to offer in-person sessions for vaccinated patients and telehealth session.
About You
Insurance and Billing Information
In-Network Plans
We are in-network with Blue Cross Blue Shield PPO, Aetna PPO and United Healthcare PPO (including UMR PPO and OPTUM PPO) for behavioral health. We handle all claims and billing directly with these insurers. You may still be responsible for deductibles, co-pays, or co-insurance as per your policy.
Out-of-Network Plans
Self-pay / Out-of-network insurance is $190 a session, due at the time of service. You may request an Astute-provided superbill to submit to your out-of-network insurance provider for possible reimbursement (Cigna, Humana, HMO plans, etc.). Reimbursement depends on your plan’s out-of-network benefits. Please contact your insurer for details.
Next Steps
Once assigned a therapist, you’ll receive an introductory email. Please respond within 24 hours to confirm your time slot; this helps us accommodate everyone waiting for appointments.
